New Patient Registrationregister_now_blue_sign

Anyone wishing to register with the practice must live within 7.5 miles of Portaferry Health Centre. If you reside outside our boundary, you may be able to register with Loughview Medical Centre in Kircubbin or Ballywalter Surgery.

If you are transferring within GP or NI, you will complete a HS200 form and if you are coming from outside the UK you will need to complete a HSCR1 form. You will also be asked to complete a patient lifestyle questionnaire. If possible, you should bring a copy of any repeat medication that you are currently prescribed.

Patients are registered with the practice and not an individual GP.

Please note you will be required to provide proof of address by way of a recent utility bill and one form of photographic ID such as a passport or driving licence.
